I know, I was surprised to see that question here too, but thought I'd answer it anyway, I read the answer on wikipedia a long time ago and thought that was the likely final nail. Its a fairly pertinent question as well, without that particular incident the competition between the front line of Thrash metal wouldn't have been as fierce and a lot of the music we know and love today may never have been written.

Dave Mustaine, or "Megadave" as he is sometimes known, was a major contributor to earlier Metallica Albums "Kill...- Master..." as the lead guitarist before Kirk was drafted in, Dave was fired due to his heavy drinking and drug use, the final straw came after a brawl which he started with Kirk and Lars after his dog scratched the paintwork on Kirk's car.
